Format a la Royal Academy of Dramatic Art, el 1962 debutà en el teatre, i també en el cinema com a actor secundari en el film The Wild and the Willing, de Ralph Thomas. Els anys següents consolidà el reconeixement com a actor teatral, entre d’altres amb les obres Little Malcolm and His Struggle Against the Eunuchs (1967, la versió cinematogràfica de la qual, dirigida per Stuart Cooper, també protagonitzà el 1974) i The Caretaker, de Harold Pinter (1972), i també amb films com ara A man of All Seasons (1966), de Fred Zinneman o 10 Rillington Place (1971), de Richard Fleischer.
Fou, però, a la televisió que aconseguí una projecció definitiva, especialment com a protagonista del telefilm The Naked Civil Servant (1975), de Jack Gold, i com a Calígula de la sèrie I, Claudius (1976). Altres interpretacions destacades en cinema són Midnight Express (1978), d’Alan Parker, pel qual obtingué un Globus d’Or; Alien (1979), de Ridley Scott; The Elephant Man (1980), de David Lynch, premi BAFTA al millor protagonista; Nineteen Eighty-Four (1984), de Michael Radford; The Field (1990), de Jim Sheridan; Love and Death on Long Island (1997), de Richard Kwietniowsky; The Proposition (2005), de John Hillcoat; An Englishman in New York (2009), de Richard Laxton, i Only Lovers Left Alive (2013), de Jim Jarmusch. L’any 2004 rebé el títol de comanador de l’orde de l’Imperi Britànic.
